naviHealth and Security Health Plan Announce Extension of Partnership to Enhance Post-Acute Outcomes

naviHealth and Security Health Plan of Wisconsin, Inc., today announced a multi-year extension of their collaboration in managing post-acute care services for Security Health Plan’s Medicare Advantage and commercial members.

The agreement, initially signed in March 2014, will continue to leverage naviHealth’s decision-support technology and field-based clinicians to deliver high-touch care management services to Security Health Plan members receiving skilled nursing facility care, long-term acute care and inpatient rehabilitation services.

naviHealth’s care managers have engaged and assessed Security Health members during their hospital stays to educate members and caregivers on care and rehabilitation options. Post-discharge, naviHealth works with patients and care teams within post-acute facilities to update care plans and monitor progress against personalized goals.
