The agreement, initially signed in March 2014, will continue to leverage naviHealth’s decision-support technology and field-based clinicians to deliver high-touch care management services to Security Health Plan members receiving skilled nursing facility care, long-term acute care and inpatient rehabilitation services.

Post-discharge, naviHealth works with patients and care teams within post-acute facilities to update care plans and monitor progress against personalized goals.
