Hospital for Special Surgery Hospital Launches the First FDA Approved Digital Pathology Platform

Hospital for Special Surgery (HSS), the world’s leading academic medical center specializing in orthopedics, rheumatology, and related disciplines, has become the first facility in the country to launch a cutting-edge technology for the digitization of pathology slides allowing pathologists to use digital images as primary means of diagnosis.

The system, called the Sectra Digital Pathology Solution, lets HSS pathologists scan and upload slides into a single repository where others in the care team โ€” from orthopedic surgeons and radiologists to rheumatologists and endocrinologists โ€” can view them, along with radiologic images that also are housed in the system. This facilitates access for specialists from across HSS to share and discuss tissue samples in real time, making care more efficient and improving clinicians’ ability to diagnose and treat their patients. Additionally, HSS clinicians and others in the care team can review the digital slides individually, simultaneously or slides shared by other hospitals to consult on complex cases.

“HSS is the first hospital in the United States to have successfully integrated a high-resolution digital scanner [Leica] with the pathology Laboratory Information System [Epic/Beaker] and the enterprise-wide PACS [Sectra] in a way that allows pathologists to use digital images for primary diagnosis,” said Thomas W. Bauer, MD, PhD, pathologist-in-chief at HSS, who led the digital pathology initiative.

Allowing surgeons, radiologists, and other physicians to view high-resolution digital images of scanned microscope slides can significantly improve patient care, Dr. Bauer said. “Access to digital pathology images, side by side with radiology images on the same enterprise-wide PACS, enables correlation between different types of studies and promotes efficient discussion about complicated cases.”

In addition, instantly viewing digital images from previous procedures will be much easier than retrieving old microscope slides from long-term storage for re-review, he added.

“The implications of the move to digital pathology are significant for patients everywhere,” Dr. Bauer said. For example, because HSS pathologists are experts in musculoskeletal pathology, they frequently receive cases for consultation from other hospitals.

“Receiving digital images electronically from hospitals with their own scanners will facilitate consultative review, and has the potential to greatly reduce turnaround time,” he added.

Digital pathology is the natural next step in the digital transformation of healthcare,” said Douglas N. Mintz, MD, a radiologist at HSS who specializes in musculoskeletal imaging and who was instrumental in developing the platform with Dr. Bauer. “We can compare what we see on an image with what was seen on biopsy. It also facilitates the correlation between radiology and pathology, which is particularly important in the diagnosis and treatment of musculoskeletal neoplasms.”

Array of High-Tech Tools

“The scanned images are extremely high quality and, when used in combination with the Leica scanner and Sectra viewer, are considered by the FDA to be equivalent to conventional microscopy for making a primary diagnosis,” explained Dr. Bauer.

HSS, ranked #1 in the world for Orthopedics by Newsweek and has been ranked #1 in the United States for orthopedics for 11 consecutive years by U.S. News & World Report, began its digital pathology initiative in 2017, and started working with Sectra technology in 2018, when it was still in the development phase. Initially, the goal was to use the system in a limited fashion, for particularly challenging cases in which clinicians had questions about pathology reports. But the COVID-19 pandemic prompted the FDA to grant the company emergency use authorization for its imaging platform, allowing HSS to speed up its transition to digital pathology.

“It is gratifying to see our vision of Integrated Diagnostics come to fruition after a long software development and integration journey,” said Inderpal Kohli, assistant vice president, IT Clinical Applications at HSS. In addition to other benefits, the solution offers HSS clinicians a familiar user interface and workflows, enabling ease of use and broader adoption across the enterprise.

Beyond patient care, digital pathology will also have an immediate impact on student and resident education, both at HSS, where the technology already is being used to help trainees, and elsewhere.

“We and other hospitals anticipate incorporating digital pathology into a variety of educational conferences available online, in which residents and fellows learn from more senior faculty by reviewing digital images,” Dr. Bauer said. “As other hospitals convert their systems to digital imaging, they will be able to better take advantage of the expertise of the faculty at HSS and other centers of excellence, thereby promoting physician education and improving the care of patients around the world.”

Wearable Devices Monitoring A Patientโ€™s Condition Reduces Risks and Saves Money
When a patient falls and injures themselves, there is a significant likelihood of that same patient falling again in the future. According to a WHO Global Report on falls prevention in older age, 34% of those over 65 are at risk of falls, raising significant concerns regarding the future demands of the health and care services.

ARMED utilizes wearable devices to monitor a patientโ€™s condition 24/7 and then uses this data to feed into their machine learning models. HAS was in search of a solution that would allow ARMED to help escalate risks in advance and create the triggers to allow for a quicker and more accurate diagnosis based on the early warning signals the data provided. They were keen to provide further customization to their solution and, with an aging population, it became imperative to also facilitate battery management.

Overhauling Manual Processes with Enterprise Mobility Management
The ARMED platform extracts data from smart watches, manufactured by Polar. With a customized SOTI MobiControl application controlling the smartphone, the Polar is paired with a smartphone via Bluetooth and the data is transferred from the watch to the phone. This data is captured, and Medical Grade Device machine learning algorithms are applied to forewarn of medical issues and changes in the behaviour of elderly patients. This also helps healthcare practitioners to prioritize resources accordingly.

SOTI MobiControl is customized to remind the patient to put their phone and watch on charge via voice alerts if their battery is below 50%. The platform then monitors the connection between the phone and watch, guaranteeing the data is constantly being transferred. Finally, SOTI MobiControl allows patients to open or restrict certain apps and communicate on the phone without needing to interact with the device.

COVID-19 frontline healthcare workers at risk of mental health problems

Medics.Academy โ€“ a revolutionary UK company delivering global access to world-leading medical education and the Ethiopian Medical Womenโ€™s Association (EMeWA) have signed a partnership agreement to help women physicians in Ethiopia.

The project will help EMeWA โ€“ an organisation established by female physicians in Ethiopia โ€“ to fulfil its vision to establish an excellence center for women physicians through one of its main thematic areas of professional development.

As part of the collaboration Medics.Academy will be investing ยฃ250,000 GBP (over 12 million Ethiopian Birr) in building a new learning community to provide digital access to training and support a target of 70% of female physicians in Ethiopia over the next 5 years โ€“ identified by the World Health Organization (WHO) as one of the 57 countries in the world with a chronic shortage of health workers.

The unique collaboration has been endorsed by the Ministry of Health of Ethiopia and, by championing access to online education, aims to drive digital transformation to help address the countryโ€™s health workforce needs, promote professional development and achieve greater representation and leadership of women in medicine.

Globally, the COVID-19 crisis has led to a significant shift to digital adoption that will likely persist post-pandemic. While there has been high growth in the adoption of education technology in recent years, this move has been deeply accelerated by COVID-19. Remote learning has become a vital part of education delivery and all across the world has been enrolled into the โ€˜new normalโ€™.

Dr Alastair McPhail CMG OBE, UK Ambassador to Ethiopia, said: โ€œWe are very proud of the partnerships between the UK and Ethiopia on health. The coronavirus pandemic has stretched health systems and healthcare workers to their limits.โ€

The pandemic has also brought pressures on health systems around the world and highlighted already existing health shortages. This includes a projected shortfall of 18 million health workers worldwide by 2030. In Ethiopia, WHO has been supporting the Ministry of Health to help increase the number of health workers and improve their skills since the beginning of the pandemic.

Dr Lia Tadesse, Minister of Health of the Federal Democratic Republic of Ethiopia, added: โ€œProfessional Development of women in healthcare is an area of priority for us in Ethiopia. Even though women make up the larger proportion in the healthcare industry, they are not visible in leadership as we want them to be due to the various challenges they face to advance their professional growth. As a result, we have few women representation in places where their voice matters and their contributions are vital. This partnership will help change this by creating an innovative learning hub that will help women foster their leadership role and advance in their careers.โ€

Over the last four years EMeWA has been working on professional development, rights of women physicians and community development to bring a holistic advancement of women in the health sector. The organization has been providing training on soft skills and mentorship for women physicians in their early careers and has a webinar series where major topics in the health sector are discussed.

EMeWA has also been increasing the visibility of women physicians who are exemplary in their practice through its Celebrating Women project which served as an inspiration for many and highlighted the increasing role of women in the healthcare system of the country.

Going forward, Medics.Academy will be working with the organisation to deliver tailored education programmes that can drive improvements in skills and diversify the countryโ€™s healthcare workforce.

Bosnia begins Covid-19 vaccination roll-out with Russian vaccine

Johnson & Johnson Pauses Covid Vaccine Trial As Participant Falls Ill

Bosnian health workers received their first Covid-19 vaccine shots on Friday and officials in the country’s autonomous Serb Republic announced a mass inoculation drive for later this month.

Bosnia had not received any vaccines until last week, when its first batch of Russia’s Sputnik V vaccine arrived, enabling it to start inoculating medical staff working in Covid-19 hospitals and departments in its Serb-dominated region.

In January, the Balkan country announced plans to order Russian and Chinese-made vaccines, fearing deliveries ordered under the COVAX scheme for poor countries and from the European Union would be too little too late.

Bosnia, which also comprises the Bosniak-Croat Federation that has not received any vaccines yet, has ordered 1.2 million vaccine doses under the COVAX scheme and nearly 900,000 from the EU. Officials hope the first doses will arrive later this month.

“We expect that Republika Srpska will receive the first quantity of vaccines for 100,000 people in the second half of February, and kick off a mass inoculation with the Russian vaccine,” Serb Republic Health Minister Alen Seranic said in the region’s main city of Banja Luka.

The region has ordered 400,000 doses of the Sputnik V vaccine and Serbia has donated vaccines for 5,000 Bosnian Serb medical staff who will be inoculated in Serbia, he said.

As of Thursday, Bosnia had recorded 125,123 cases of the new coronavirus and 4,868 deaths.

In the Western Balkans region, inoculations have begun only in Serbia, which ordered the Pfizer/BioNTech , Sputnik V and Sinopharm vaccines. Serbia has also announced it could have the capacity to manufacture Sputnik V domestically by the end of the year..

Montenegro, North Macedonia and Kosovo have not received any vaccines yet. Albania has received some. In Montenegro, municipalities along the Adriatic coast have been hit hard by Covid-19, with 118 new cases alone in the coastal town of Budva. It has reported 66,857 coronavirus cases and 861 deaths. The system is designed to provide accurate and controlled limb lengthening, with more than 3,500 cases performed in 15 countries since its development. With appropriate preoperative planning, it allows achievement of axial and torsional bone alignment intraoperatively, as part of the limb-lengthening procedure.

The FITBONE intramedullary lengthening system is available in the U.S. under a U.S. Food and Drug Administration 510(k) clearance and in European Countries under a CE Mark approval.
