Medtronic Temporarily Modifies Several Cardiopulmonary Product Indications for Longer Duration Use in ECMO Therapy to Address COVID-19

During this critical time, Medtronic remains committed to our strategy of delivering critical care products designed and indicated for Extracorporeal Membrane Oxygenation (ECMO) to treat patients around the world. Following guidance issued by the U.S. FDA on April 6, 2020, several Medtronic cardiopulmonary technologies are now permitted to temporarily be used in the U.S. for ECMO therapy greater than six hours.

Under this new guidance the FDA is allowing temporary limited modifications to the indications of certain FDA-cleared or FDA-approved cardiopulmonary devices without prior submission of premarket notification. These modifications are allowed, considering the public health emergency, when they do not create an undue risk. This limited indication modification for ECMO therapy greater than six hours has not been cleared or approved by the FDA and is in effect only for the duration of the public health emergency related to COVID-19 as declared by the Department of Health and Human Services (HHS).

The devices listed below are currently FDA-cleared for extracorporeal support for periods appropriate to cardiopulmonary bypass procedures (up to 6 hours).

In accordance with the new FDA guidance, the devices listed below can now be used for longer than 6 hours in an ECMO circuit to treat patients who are experiencing acute respiratory/cardiopulmonary failure during the COVID-19 Public Health Emergency*.

Siemens Healthineers announced the U.S. FDA has issued an Emergency Use Authorization (EUA) for its laboratory-based total antibody test to detect the presence of SARS-CoV-2 antibodies including IgM and IgG in blood. Test data demonstrated 100 percent sensitivity2 and 99.8 percent specificity. The total antibody test allows for identification of patients who have developed an adaptive immune response, which indicates recent infection or prior exposure. Testing can begin immediately with more than one million tests already shipped to health systems and laboratories.

A spike protein on the surface of the SARS-CoV-2 virus enables the virus to penetrate and infect human cells found in multiple organs and blood vessels. The Siemens Healthineers Total Antibody COV2T assay was thoughtfully designed to detect antibodies to the spike protein. These antibodies are believed to neutralize the SARS-CoV-2 virus and therefore prevent infection. Multiple potential vaccines in development for SARS-CoV-2 include the spike protein within their focus.

Tests with high level of specificity yield low false positive rates, which is highly desirable especially when prevalence of disease is low. For example, a test with 99.8 percent specificity, in populations with low disease prevalence (e.g., 5 percent), will yield a positive predictive value (PPV) of 96.5 percent. In other words, 96.5 people in 100 testing positive will truly have antibodies. This number only goes up as the disease prevalence increases; for example, in communities with 10 percent of disease prevalence, this test would yield a PPV of 98.3 percent, meaning that 98.3 people in 100 people with positive tests results have antibodies. High-quality antibody testing is vital to accurately assess the prevalence of disease, which varies across the U.S.

The total antibody test SARS-CoV-2 Total (COV2T)3 is available on the largest installed base of high-throughput analyzers, including the Atellicaยฎ IM immunoassay analyzers, which can run up to 440 tests per hour1 and enables a result in just 10 minutes. The test also is available on the companyโ€™s expansive installed base of ADVIA Centaurยฎ XP and XPT analyzers, which can test up to 240 samples per hour with a result in 18 minutes.

Comparable antibody tests4 for Siemens Healthineers Dimension Vistaยฎ and Dimensionยฎ EXL systems started shipping on May 29, further extending clinical reach across the companyโ€™s 20,000 systems installed worldwide.

The tests were developed in the epicenter of the pandemic, are manufactured in Walpole, Mass. and Newark, Del., and are distributed from Plainfield, Indiana. The company is prepared to ramp up production as the pandemic evolves with capacity exceeding 50 million tests per month across its platforms starting in June.

Boston Scientific announced the U.S. launch of the DIRECTSENSE Technology, a tool for monitoring the effect of radiofrequency (RF) energy delivery during cardiac ablation procedures. Available on the RHYTHMIA HDxโ„ข Mapping System, the DIRECTSENSE Technology, which received U.S. FDA approval in April, is the only tool to monitor changes in local impedance โ€“ electrical resistance โ€“ around the tip of the INTELLANAVโ„ข MiFi Open-Irrigated (OI) ablation catheter, offering physicians an additional measurement of therapy effect during an ablation.

Ablation is a treatment option for patients with cardiac arrhythmias in which physicians use a catheter to create lesions and destroy heart tissue that causes abnormal rhythms. The DIRECTSENSE Technology provides data on the impedance around the catheter tip to measure the ability of the tissue to respond to RF energy before physicians deliver therapy. During ablation, the tool tracks the change in local impedance which, in conjunction with other measures, offers physicians a distinct understanding of tissue characteristics and how they are affecting that tissue. These insights may indicate temperature change in the tissue, helping to reduce the chances of over-ablation and avoid complications.

“Knowing the change in impedance around the tip of the catheter provides unique information about local tissue characteristics and the development of the lesion,” said David J. Callans, M.D., professor of medicine at the Perelman School of Medicine, University of Pennsylvania โ€“ the first center to use the new technology in the U.S. “Unlike existing products on the market, the DIRECTSENSE Technology assists physicians in evaluating pre-ablation tissue health, while supporting better clinical understanding of how they are influencing that tissue to guide minimal, predictable ablation during a procedure.”

According to chronic data from the LOCALIZE clinical trial, a retrospective analysis of the DIRECTSENSE Technology presented by Ignacio Garcia-Bolao, M.D., director of cardiology and cardiovascular surgery, University of Navarra, Pamplona, Spain at Heart Rhythm Society 2020 Science, a local impedance decrease of โ‰ฅ16.6 ohms with an inter-lesion spacing of โ‰ค 6mm showed a โ‰ฅ 98% positive predicative value of durable pulmonary vein block at three months in patients with paroxysmal atrial fibrillation (AF).

RaySearch Laboratories AB (publ) and Varian Medical Systems have entered into an interoperability agreement that will allow the RayCareยฎ* oncology information system to connect to Varian TrueBeam linear accelerators.

The ability to schedule radiation treatments and keep track of the treatment progress is a key functionality in an oncology information system. Under the announced agreement, a dedicated interface has been defined which allows RayCare and Varian TrueBeam to communicate the data necessary for an integrated workflow. This means that clinics will be able to use RayCare to manage all clinical activities around their Varian TrueBeam accelerators.

The development of the interface is already ongoing, and the target is to release it before the end of 2020. Commercial availability will be subject to regulatory clearance in some markets.
