Enterprise imaging continues to evolve with more capabilities than ever before as imaging departments strive to deliver medical imaging securely, with the right tools at the right time to the right patient. While these imaging advancements provide many more opportunities, it is also growing increasingly complex with even more disparate processes that must be maintained, supported and standardized across an entire enterprise environment.

Many of these inefficient processes can be avoided by improving workflows or leveraging technology โ€“ although that can be easier said than done. One approach that is helping healthcare executives and department heads move their organizations forward through technology is the use of adoption and maturity models, which are designed to help organizations progressively measure and advance analytics capabilities to improve healthcare delivery. The informatics revolution has helped place patients where they belong: at the heart of the healthcare system. Health systems are increasingly seeing the shift from systems of record โ€“ a static file of patient information, to systems of engagement โ€“ in which the patient, and all the staff they interact with, are supported to play a greater role in managing the care experience.

Why Digital Twins are a Critical Part of Digital Healthcare

Introduction:

Digital twins are often applied to industrial assets, but the healthcare industry can leverage the concept of living model and apply it to patient wellness. This article discusses how the data we generate is a critical part of digital healthcare.

As the connected society continues to be the platform on which we work, rest and play, there is a defined need for every industry to not only leverage the digital engineering tools available but also use them to their best advantage. The convergence of the physical and digital worlds in differing business sectors has been inevitable for quite some time, but the shift from a reactive attitude to a proactive stance is now becoming commonplace.

We are all too familiar with how traditional industries such as banking and finance, manufacturing, the supply chain and retail have all benefited from increased digitalization, but the requirement for data-driven and digital insights has made waves in the healthcare and life sciences sector. Nowhere is this more evident than in the adoption of digital twins in patient care and wellness.

For anyone who is unfamiliar with the concept, a digital twin is a virtual real-time representation of a physical asset or process. A cloud-based solution, the digital version of a physical element utilizes the millions of data points being generated by said assets or processes and allows for the creation of what can be seen as living model of what is happening in real-time.

To date, digital twins have been used primarily for industrial assets or machinery, but the possibilities in terms of digital healthcare solutions go beyond simply knowing when a generator will fail. If we are being more specific, a digital twin of a human being could literally be the difference between life and death.

The digitalization of patient care

The question that needs to be asked, therefore, is how a virtual version of a living person can accurately provide the actionable insights that a medical professional will need to effectively administer care. And, if data is the key to patient wellness, can healthcare providers both treat and (in theory) cure diseases before they become a problem.

The healthcare industry is not a newcomer to the benefits of digital information or algorithm-centered insights. Real-world digital applications such as internet-connected medical devices, wearables and implantables have been available for some time, but the digital twin raises the bar on several levels. Importantly, the digitalization of care is now increasingly cloud-based, with providers using software to validate the situations that are part of their day job.

In fact, researchers and providers have seen the value of a healthcare data analytics platform rise in recent years, driven (in part) by the fact that patients have so many data points to study. What has become clearer to clinicians is that disease can not only be treated with the right information at the right time (often using sensors), but also prevented from becoming a life-threatening event.

If we think about a condition such as, say Chronic Obstructive Pulmonary Disease (COPD), then the use of digital twins to treat the patient becomes a game-changer. According to the World Health Organization, COPD is the third leading cause of death worldwide and responsible for the deaths of around three million people every year.

The key point to remember here is that COPD is not curable per se, but treatable. That means that a patientโ€™s suffering can be alleviated by the application of antibiotics, steroids or nebulized medicine. By integrating a digital twin into the wellness assessment and subsequent preventative processes, a doctor or physical care provider can see where the lung disease has taken hold and act accordingly.

This can be demonstrated in the following scenario:

A person suffering from shortness of breath (a classic symptom of COPD) is admitted to a hospital for diagnosis. Before that patient even gets seen by a doctor, data relating to the condition of that individual is entered as an electronic health record (EHR). High-quality sensors that monitor the state and health vitals are then attached โ€“ ECG, air quality, breath straps, temperature and spirometer, for example. The system then pulls data from other life events relating to the personโ€™s medical history into the EHR, generating a digital twin from the historical and real-time data.

The patient is now transmitting data that can be studied, analyzed and acted upon. The digital twin is providing all the information that is required and, in many cases, can accurately predict the results of the various medications administered or even draw on very granular information such as food allergies or similar.

Without the information being provided by the virtual version of the person in front of them, doctors are relying on their expertise to make the right decisions, a situation that is not always possible when time is of the essence.

Digital Twin = Actionable Health Insights

As we have noted above, the digital twin is a living model of a physical object. For the twin to be successful, it must work with that object in such a way that the virtual and real-world versions exhibit perfect synchronicity.

When you apply the concept to โ€œdumbโ€ assets such as machinery, planes, trains and automobiles, then the value of the data generated prevents unintended down time, costly repairs and, importantly, the need to find a replacement. For the record, NASA was responsible for the first practical application of a digital twin back in 2010, when the agency demonstrated how the life support issues with Apollo 13 could have been solved on the ground.

Flip the advantages of a digital twin to a human being and the benefits of knowing exactly what is going on at any one time become a critical part of both ongoing treatment and preventative care. A recent study by researchers at Cambridge University is doing exactly that, using AI and mathematical modelling to โ€œprovide a panoramic view over current and future pathophysiological conditionsโ€ โ€“ in other words, both predicting health issues over time and personalizing healthcare in a digital space.

Data is a commodity that is used by many companies to understand what the customer wants and when they want it, so it follows that the use of these zeros and ones to save lives โ€“ or, at the very least, alleviate suffering โ€“ will go a long way towards cementing the future of digital healthcare.

Medical professionals have always used data to understand, prevent and cure disease, but that often relies on the combination of learned knowledge and being able to assess the physical state of a patient. A digital twin augments that knowledge and allows for better decision-making. Digital healthcare is both our present and the future, so being able to see the full patient picture is going to be both a logical step forward and, importantly, a literal life saver.

Launch of world-first expert resource to foster best practice osteoporosis care in the Asia Pacific

More than 50 per cent of the world’s hip fractures are expected to occur in the Asia Pacific by the year 2050. In a strong effort to stem the tide, and to promote quality care in osteoporosis, the Asia Pacific Consortium on Osteoporosis (APCO) has launched a world-first, interactive, educational osteoporosis resource โ€“ the APCO Health Care Professional (HCP) Peer to Peer Educational Modules โ€“ today, to mark World Osteoporosis Day.

The survival of those who sustain an osteoporotic fragility fracture is significantly compromised for up to six years, with patients facing twice the risk of death within the first year. Furthermore, a prior fracture at any site doubles a person’s risk of refracture. Yet despite these alarming statistics, more than 80 per cent of fragility fracture patients are neither assessed, nor treated, placing a substantial, but importantly, preventable burden on already strained healthcare systems. This is despite the extensive availability of safe and effective osteoporosis management strategies.

Minimum clinical standards for the assessment and management of osteoporosis are therefore urgently required in the Asia Pacific, to inform clinical practice guidelines, and improve osteoporosis care. It was with this aim in mind that APCO launched โ€˜The APCO Framework’ in January, 2021. โ€˜The Framework’ comprises 16 minimum clinical standards that serve as a benchmark for the provision of best practice osteoporosis care in the Asia Pacific.

The APCO HCP Peer to Peer Education Modules will offer physicians a comprehensive, evidence-based resource encouraging widescale implementation of the minimum clinical standards of care for osteoporosis advocated in The APCO Framework. This will enable the delivery of best practice osteoporosis care throughout the Asia Pacific โ€“ the world’s fastest ageing region.

According to APCO Chairperson, and Director of the Osteoporosis and Bone Metabolism Unit, Singapore General Hospital, Dr Manju Chandran, Singapore, there are significant inconsistencies in clinical practice guidelines in the Asia Pacific for the management and prevention of osteoporosis and fragility fractures. These guidelines vary extensively in scope and recommendations. There is also a lack of information available on adherence to national guidelines in daily clinical practice.

“It is to address this care gap, that APCO has developed an interactive, 17-module educational series today, to arm osteoporosis champions in the Asia Pacific, with information, supporting data, topical literature summaries, and best practice examples that support and emphasise each of The APCO Framework’s 16 minimum clinical standards and emerging themes in osteoporosis care.

“Increasing awareness and education among healthcare professionals in the Asia Pacific is imperative to bringing change to the real-world clinical practice of osteoporosis care and fracture prevention,” said Dr Chandran.

“The Asia Pacific is home to 4.5 billion people, rapidly ageing populations, and vastly different healthcare systems.

“An overwhelming 319 million people aged 50 years and over from the Asia Pacific are projected to be at high risk of osteoporotic fracture over the next two decades, while more than half of the world’s hip fractures are expected to occur in the region by 2050,” Dr Chandran said.

“Singapore, where I live, for instance, has one of the highest hip fracture rates in the world, with a four-to-five-fold increase in incidence noted over a recent 30-year period.

“The incidence of osteoporotic fractures in Singapore is projected to rise from more than 15,000 in 2017, to 24,100 by 2035, an increase of 57.9 per cent. The total economic burden (including both direct and indirect costs) associated with these fractures is projected to grow from SGD 183.5 million (USD 214 million) in 2017, to SGD 289.6 million (USD 136 million) by 2035.”

After one year, two in five people who sustain a hip fracture will be unable to walk independently, while two-thirds will encounter difficulty performing at least one essential daily task.

“Economically, the burden of current treatment of a single hip fracture in the Asia Pacific is extremely high, equating to approximately 19 per cent of our region’s per-head gross domestic product (GDP),” said APCO member, Consultant Endocrinologist at P. D. Hinduja National Hospital and Medical Research Centre and Hope & Care Centre, Dr Manoj Chadha, Mumbai, India.

In India alone, the direct medical costs of hip fractures was an estimated USD 256 million in 2018, the costs for which are projected to increase to USD 612 million by 2050.

“In 2013, 50 million people in India were either osteoporotic, or considered to be living with low bone mass. The 2018 AFOS study predicted the number of hip fractures in India would more than double from approximately 332,000 in 2018 to 792,000 in 2050,” Dr Chadha said.

“Given the anticipated exponential growth in fragility fractures due to the Asia Pacific region’s rapidly ageing population, mass urbanisation, and our increasingly sedentary lifestyles, we must act now.

“Osteoporosis experts can use the new APCO HCP Education Modules either in full, or in part, to inspire and further inform their peers and professional communities on those at high-risk for fracture who require identification and assessment, based on their medical and fracture history, risk factors, co-occurring diseases, country-specific osteoporosis screening tool results, and falls risk.

“Supporting data on the administration, duration, monitoring of, and adherence to, various treatment interventions and their associated side-effects, as well as the critical need for long-term management plans, also feature in the APCO modules,” said Dr Chadha.

Osteoporosis is vastly under-diagnosed and under-treated. Disturbingly, millions of people worldwide at high risk of fracture remain unaware of this underlying, silent disease. Championed by the International Osteoporosis Foundation (IOF), World Osteoporosis Day is dedicated to raising global awareness of the importance of prevention, diagnosis and treatment of osteoporosis and musculoskeletal diseases.

“This World Osteoporosis Day, we encourage healthcare providers, policy makers, patients, members of the public, and the media on a global, regional and national stage, to recognise the monumental human and socio-economic burden of osteoporosis, and the severe impact of fractures on a patient’s independence and quality of life,”

IOF Chief Executive Officer, and APCO Executive Committee member, Dr Philippe Halbout, Switzerland said.

“One of the emerging osteoporosis themes outlined in the APCO HCP Education Modules, is the importance of Asia Pacific-wide systemic integration of case identification and management at all levels of health systems, including acute care services, when patients present with fractures through Post Fracture Care Coordination Programs, such as Fracture Liaison Services (FLS).

“These coordinated systems of care aim to identify, treat and monitor patients presenting with a fragility fracture. Intervention can reduce fractures by up to 50 per cent, deliver substantial financial savings, and ultimately, save lives,” said Dr Halbout.

“Today’s launch of the APCO Education Modules demonstrates the organisation’s ongoing commitment to achieving greater consistency in national and regional clinical practice guidelines for the screening, diagnosis, and management of osteoporosis in the Asia Pacific.”
